Application of Gene Therapy in Lung Cancer: Current Strategies, Clinical Advances, and Future Perspectives

Context: Lung cancer remains the leading cause of cancer-related mortality worldwide. Despite advances in conventional therapies, overall survival remains limited, highlighting the need for novel interventions. Gene therapy has emerged as a promising strategy to target the molecular and genetic drivers of lung cancer, offering opportunities for precision treatment.
Evidence Acquisition: This review comprehensively examines gene therapy for lung cancer, covering its molecular and genetic underpinnings, fundamental principles, current strategies, clinical progress, combination therapies, challenges, and future directions. A literature-based review was conducted, focusing on preclinical and clinical studies related to gene therapy in lung cancer. Key databases and recent publications were examined to summarize evidence on tumor suppressor restoration, oncogene silencing, genome editing, oncolytic virotherapy, immune modulation, and combination strategies with chemotherapy, radiotherapy, or immunotherapy. Results: Gene therapy strategies in lung cancer include tumor suppressor gene delivery (p53, TUSC2), RNA interference targeting oncogenes (KRAS, EGFR), CRISPR/Cas9-based genome editing, and immunomodulatory approaches including CAR-T cells. Clinical trials have demonstrated safety and preliminary efficacy, particularly in advanced non-small cell lung cancer. Combination approaches with chemotherapy, radiotherapy, and immune checkpoint inhibitors show synergistic effects. Key challenges include delivery efficiency, tumor heterogeneity, immune reactions, and safety concerns. Future directions emphasize personalized therapy, improved vectors, precise genome editing, multimodal combination strategies, and real-time monitoring tools.
Conclusions: Gene therapy represents a promising frontier in lung cancer management. While significant hurdles remain, advances in molecular understanding, delivery systems, and precision medicine approaches suggest that gene-based interventions may become integral components of future lung cancer treatment paradigms.
